Why fibromyalgia may be connected to your upper neck
Fibromyalgia is a real and often exhausting condition. It brings widespread pain, deep fatigue, poor sleep and trouble thinking clearly. If you live with it, you may have heard that your tests are normal, or been told to manage it with medication, exercise and stress reduction. Those steps can help, but they do not answer a question many patients ask: why is my body hurting so much in the first place?
Many researchers now describe fibromyalgia as a problem of central sensitization. In simple terms, the nervous system becomes over-sensitive and starts treating ordinary signals, such as light pressure or normal muscle use, as pain. It is a bit like a smoke alarm that goes off every time you make toast. The alarm is working, but the setting is too high.
This is where the upper neck may matter. Your brainstem sits just above the atlas, the top bone of your neck. The brainstem helps manage pain signals, sleep cycles, heart rate and your body’s stress response. When the atlas shifts out of its normal position, often after an old injury such as a car accident or fall, it can change how the head balances on the neck. The body compensates with tight muscles, uneven shoulders and strain down the spine.
Our view is that this ongoing strain may add stress to the brainstem area and keep the nervous system on high alert. For some people with fibromyalgia, correcting the atlas may be one way to help the system calm down. It is not the only factor, and it does not apply to everyone.
We want to be honest about the limits. There is no cure for fibromyalgia, and NUCCA care is not a treatment for the condition itself. Our goal is to find out whether an upper neck misalignment is a contributing factor in your symptoms, and if it is, to correct it gently and precisely.
Fibromyalgia, sleep and fog
Poor sleep and fibromyalgia feed each other. Pain makes it hard to sleep, and poor sleep makes pain feel worse the next day. The mental fog many patients describe often follows the same cycle. Because of this, we pay close attention to your sleep and energy, not just your pain. You can read more about these connections on our pages about sleep help and brain fog and fatigue.
How NUCCA care helps fibromyalgia
NUCCA (National Upper Cervical Chiropractic Association) care focuses only on the alignment of the atlas. It is precise, measured and very gentle. You can learn more about the method on our NUCCA upper cervical care page. For fibromyalgia, care follows three steps.
1. We measure first. We start with a full health history, including when your symptoms began, past injuries, sleep, energy and what makes your flares better or worse. Non-invasive exams check your posture and balance. Precise imaging of the upper neck then shows exactly how your atlas is positioned, down to fractions of a degree. We do not guess.
2. We correct gently. You lie comfortably on your side while the doctor applies a light, sustained pressure just behind the ear. There is no twisting, no popping or cracking, and no forceful pressure on tender points. For a body that is already hurting, this matters. Most patients describe the correction as surprisingly light.
3. We monitor how well it holds. After the correction, we recheck your alignment. At follow-up visits, we track your pain, sleep, energy and flare-ups alongside your measurements. A correction that holds gives your nervous system time to settle, and we only correct again when your measurements show the atlas has shifted. Progress with fibromyalgia is often gradual, and we pace care to your body.
Gentle care for a sensitive body
We know that many people with fibromyalgia have had care that left them sorer than before. That is why we keep each visit calm and unhurried. Nothing is done without explaining it first, and you can ask us to pause at any point. After an early correction, some patients notice a short-lived healing response, such as mild tiredness or muscle soreness, as the body adapts to a new alignment. We will talk you through what to expect and how to rest well on the days after a correction.
NUCCA care works best as one part of a broader plan. We encourage patients to keep working with their physician, and to continue the gentle movement, sleep habits and stress management that help them. Please do not stop or change any medication without talking to the doctor who prescribed it.
What the research says
Research on upper cervical care for fibromyalgia is still developing. Our previous website described studies suggesting that upper neck misalignment may contribute to central sensitization.1 It also described reports of patients receiving upper cervical care noticing improvements in pain, sleep, energy and quality of life.2 We are confirming the full references for these studies and will update this page when we have them.
Until then, here is what we can say honestly. The idea that fibromyalgia involves an over-sensitive nervous system is widely accepted in medicine. Whether correcting the upper neck helps any one person is something we can only find out by measuring carefully and tracking your response. In more than 30 years of care, we have seen some patients with fibromyalgia report meaningful changes and others report less. We will tell you what we find at each step.
